(i)     under a Work Permit or awaiting the determination of an

The following are the documents we require:

2.      Copies of passports of parent(s) and child(ren);
% r% A, z9 A' G% `6 a9 z3.      Birth certificate of child(ren) / Family Register (translated

4 Y5 `; i' |7 b% d; X9 }4.      Proof of residential address / tenancy (eg lease agreement/tax
bill/utility bill - driver’s license not acceptable);
6 t3 {& q- {" A9 m3 Z- m& s1 i% q5.      Documentation to verify child(ren)’s first ever date of entry
into Canada (eg. passport stamp / immigration documents / airline
. l  T% \& a+ K1 h- ^! K0 `ticket etc.);
6.      Copies of all immigration documents received ie. Work Permit,
* J% t4 I/ g5 |Study Permit,  Visitor Records and any other documents we might
require.
